RN Case Manager - Home Health (South Team) Shift Details
- Monday-Friday, 8:00 AM - 4:30 PM
- Local Contract - Nursing
- Territory covers the Plainsboro and West Windsor areas
- RN Case Manager role within a home health setting
- High-volume position — expect to see 5-6 patients per day
- High volume of documentation required
- Minimum 2+ years of home health experience
- Case management experience required to be considered
- OASIS experience required to be considered
- Wound care experience preferred
- Candidates coming from a high-volume home health role are strongly encouraged to apply
