Acute Med/Surg RN - Part Time Position Position Details • Part Time (2x12s) - day or night shift (scheduled as needed) • EOW and holiday required • Rotational D/E/N/W/Holidays Required Qualifications • NYS RN Licensure Required • BLS certification required • ACLS certification required • One (1) Year Experience serving in Med/Surg Environment as a Registered Nurse Position Overview • Apply knowledge of nursing theory, technical, and communication skills in delivering high quality compassionate care to patients and families • Assess patient need, develop a plan of care in collaboration with the interdisciplinary team, provide direct care, evaluate patient progress, and coordinate the care delivery throughout the patient's episode • Develop specialized skill in the area of cardiac care including telemetry monitoring • Provide patient family education to assist the patient and family in understanding self care beyond hospitalization, including lifestyle changes to support health and healing • Promote comfort and provide emotional support in a healing environment Nursing Care Delivery • Apply nursing knowledge and skill in delivery of care by assessing patient/family needs through active listening, social perceptiveness, and analyzing data/information obtained through direct observation and assessment, technology, or health team communication • Create a developmentally and age appropriate, culturally relevant, individualized plan of care to address the nature and scope of the health problems identified • In collaboration with the patient, family, physician, and health care team members, set goals and monitor the patient's progress • Adjust the plan based on patient response or change in condition/reprioritization of needs Quality and Professional Development • Promote, advocate for and protect the health, safety and rights of the patient • Systematically evaluate quality and effectiveness of Nursing Practice, use the results of Quality Improvement activities to initiate changes in individual nursing practice and to influence care delivery • Engage in self-evaluation of practice, identifying strengths and areas in which professional development would be beneficial and take accountability to increase knowledge and enhance skill • Use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems • Demonstrate skill in application of nursing interventions including medication administration Patient and Family Education • Motivate the patient/family by teaching them what is needed to continue care and healing at home • Assess the patient/family learning styles and structure the sessions to maximize understanding • Seek "teachable" moments during the care delivery process to emphasize pertinent information • Ask questions to evaluate retention of key points in the self care process • Provide tools and information to assist the patient family after discharge • Document the education provided and the patient/family response to the education Team Collaboration and Leadership • Encourage and build mutual trust, respect, and cooperation among team members to promote members of the team working together to accomplish the best possible patient outcome • Teach others as appropriate and delegate tasks and interventions consistent with the individual's education, training, and scope of practice • Comply with infection prevention strategies and maintain competencies required to care for the specific population in the area assigned • Report potential safety risks and inappropriate/questionable practices • Promote leadership awareness and foster positive change • Conduct rounds per protocol and use tools and equipment needed to keep the patient/family informed, engaged, and safe Patient-Centered Care • Reflect the importance of making a meaningful connection with patient/family in nursing practice • Seek to develop a helping, trusting, and caring relationship by being present in the moment of interaction • Listen, communicate, engage, and anticipate the patient/family needs • Strive to understand what is most important to the patient/family, acknowledging that the patient and family are the central focus of all care and services • Embrace the importance of creating a Healing Environment as a key element of professional nursing practice
