Acute Med/Surg RN Part Time Position Shift Details • Part Time 2x12s, day/night shift rotations, EOW and Holiday • 2x12.0 hour shifts • Rotational D/E/N/W/Holidays Required Qualifications • NYS RN Licensure Required • BLS certification required • ACLS certification required • One (1) Year Experience serving in Med/Surg Environment as a Registered Nurse Position Overview • The Registered Nurse applies knowledge of nursing theory, technical, and communication skills in delivering high quality compassionate care to patients and families • The RN assesses patient need, develops a plan of care in collaboration with the interdisciplinary team, provides direct care, evaluates patient progress, and coordinates the care delivery throughout the patient's episode • The Med/Surg RN develops specialized skill in the area of cardiac care including telemetry monitoring • The RN is accountable for patient family education to assist the patient and family in understanding self care beyond hospitalization, including lifestyle changes to support health and healing • He/She promotes comfort and provides emotional support in a healing environment Nursing Knowledge and Skill Application • Applies nursing knowledge and skill in delivery of care by assessing patient/family needs through active listening, social perceptiveness, and analyzing data/information obtained through direct observation and assessment, technology, or health team communication • Creates a developmentally and age appropriate, culturally relevant, individualized plan of care to address the nature and scope of the health problems identified • In collaboration with the patient, family, physician, and health care team members, sets goals and monitors the patient's progress • The RN adjusts the plan based on patient response or change in condition/reprioritization of needs Quality and Safety Responsibilities • Promotes, advocates for and protects the health, safety and rights of the patient • Systematically evaluates quality and effectiveness of Nursing Practice, uses the results of Quality Improvement activities to initiate changes in individual nursing practice and to influence care delivery • Engages in self-evaluation of practice, identifying strengths and areas in which professional development would be beneficial and takes accountability to increase knowledge and enhance skill • Uses logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems • Demonstrates skill in application of nursing interventions including medication administration Patient and Family Education • Motivates the patient/family by teaching them what is needed to continue care and healing at home • Assesses the patient/family learning styles and structures the sessions to maximize understanding • Seeks "teachable" moments during the care delivery process to emphasize pertinent information • Asks questions to evaluate retention of key points in the self care process • Provides tools and information to assist the patient family after discharge • Documents the education provided and the patient/family response to the education Team Collaboration and Leadership • Encourages and builds mutual trust, respect, and cooperation among team members to promote members of the team working together to accomplish the best possible patient outcome • Teaches others as appropriate and delegates tasks and interventions consistent with the individual's education, training, and scope of practice • Complies with infection prevention strategies and maintains competencies required to care for the specific population in the area assigned • Reports potential safety risks and inappropriate/questionable practices • Promotes leadership awareness and fosters positive change • Conducts rounds per protocol and uses tools and equipment needed to keep the patient/family informed, engaged, and safe Patient-Centered Care • Nursing practice reflects the importance of making a meaningful connection with patient/family • Seeks to develop a helping, trusting, and caring relationship by being present in the moment of interaction • Listens, communicates, engages, and anticipates the patient/family needs • Strives to understand what is most important to the patient/family, acknowledging that the patient and family are the central focus of all care and services • Embraces the importance of creating a Healing Environment as a key element of professional nursing practice
