Job Description
Registered Nurse – Community and Home-Based Care $90 Per Hour

Alerion Healthcare is partnering with a highly respected healthcare organization in New York to place experienced Registered Nurses into a community-based care program serving patients in their homes and outpatient settings. This is a unique opportunity to work in a supportive clinical environment while enjoying a predictable weekday schedule and the potential for long-term placement.
This position is offered on a temporary to temp-to-hire basis, giving you the chance to evaluate the role and environment while building stability and career continuity.
What Makes This Role Different
This role gives you the opportunity to practice nursing the way it was meant to be practiced. You will have time to assess, educate, treat, and truly connect with each patient. You will work independently in the field while remaining part of a collaborative clinical care team.
The schedule is Monday through Friday from 8:30 AM to 5:30 PM with 40 hours each week. A 401(k) retirement plan with company match is offered through Alerion Healthcare.
What You Will Do
You will provide hands-on nursing care to patients in their homes and outpatient care settings throughout the Bronx and surrounding areas. Your day will include wound care and dressing changes, medication administration and reconciliation, and full health and functional assessments. You will monitor patient conditions, communicate changes to the care team and physicians, and provide education to patients and caregivers related to medications, safety, and disease management.
You will document care in an electronic health record system, follow clinical and infection control protocols, and participate in care planning and interdisciplinary collaboration as part of a coordinated healthcare team.
Some nursing shifts may also take place in outpatient or medical practice settings, depending on patient needs.
Who This Role Is Ideal For
This opportunity is well suited for Registered Nurses who enjoy working one-on-one with patients and want a balance of independence and clinical support. Nurses who are confident in their clinical judgment, organized, and compassionate thrive in this role.
You should have an active New York Registered Nurse license and at least three years of clinical nursing experience, with home care or community-based care preferred. Experience working with elderly or medically complex patients is highly valued. You should be comfortable using electronic health records and able to manage multiple patient cases efficiently.
The ability to travel within the service area is important, and a reliable vehicle is preferred. Some weekend, evening, or on-call availability may be required depending on patient needs.
