The Role

• 
  Provide comprehensive primary care across acute and chronic conditions
• 
  Deliver patient-centered care with a focus on prevention and long-term outcomes
• 
  Manage chronic disease and wellness-focused treatment plans
• 
  Educate patients and families on conditions, care plans, and preventive strategies
• 
  Coordinate specialty referrals and additional services as needed
• 
  Maintain timely, accurate documentation in the EMR
• 
  Participate in ongoing clinical development and evidence-based initiatives

Requirements

• 
  MD or DO from an accredited medical school
• 
  Board eligible or board certified in Internal Medicine or Family Practice
• 
  Active, unrestricted state medical license
• 
  Active DEA license
• 
  TB clearance (or ability to obtain)
