Position Overview

We are seeking a compassionate and motivated Physical Therapist to join a dynamic rehabilitation team. In this role, you'll evaluate patients, develop individualized treatment plans, and help individuals regain mobility, improve function, and achieve their rehabilitation goals. This is an excellent opportunity for a therapist who is passionate about patient-centered care and enjoys working in a collaborative environment.
Responsibilities
  • Evaluate patients and develop individualized treatment plans based on clinical findings and physician referrals.
  • Implement evidence-based therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence.
  • Monitor patient progress and modify treatment plans as needed to maximize outcomes.
  • Educate patients and family members on treatment plans, home exercise programs, injury prevention, and wellness strategies.
  • Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory standards.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team.
  • Ensure a safe treatment environment by following infection control and safety protocols.
  • Participate in interdisciplinary care planning and discharge planning.
  • Maintain compliance with all state, federal, and organizational policies and procedures.
  • Perform additional duties as assigned to support the rehabilitation department.
Qualifications
  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the applicable state or eligibility to obtain licensure.
  • Current CPR/BLS certification (if required).
  • Strong clinical assessment and treatment planning skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
