Nurse Practitioner (Jersey City, NJ)

• Competitive Salary: $135,000 to $145,000, based on experience and credentials

Position Overview
As the Nurse Practitioner, you’ll play a critical role in coordinating participant care alongside Physicians and community primary care physicians.  In this full-time role, you’ll be the liaison between participants, healthcare providers, and plan staff, ensuring holistic, person-centered care plans are implemented, while facilitating timely, preventative care services.

What You Will Do

• Perform annual wellness assessments for participants, adjusting care for those with higher risks, frailty, or complex needs
• Facilitate same-day urgent care, timely assessments, and transitions of care upon hospital discharge
• Collaborate closely with PCPs to co-manage high-risk participants, proactively identifying and addressing gaps in care
• Ensure accurate clinical documentation to reflect participants’ health status and outcomes
• Oversee and coordinate home care services, tailoring care plans to individual needs with follow-ups for comprehensive outcomes

About You:

• Minimum of 1 year in clinical nursing practice (home care, long term care or care management preferred)
• At least 1 year of experience with elderly or frail populations
• Valid State Nurse Practitioner license, DEA Certification, and AANP or ANCC Board Certification
