South Bay, CA | Mon–Fri | No Nights, Weekends, or On-Call
$125,000–$175,000 + Bonus Potential

The Role
Provide in-home assessments and ongoing medical management
Manage acute, post-acute, and transitional care patients
Develop and execute personalized care plans
Order diagnostics, prescribe treatments, and educate patients/families
Collaborate closely with physicians and interdisciplinary teams

Requirements
MSN required
Adult or Geriatric NP certification
1+ year NP experience (home-based or transitional care a plus)
Active CA RN & NP license
TB clearance (or ability to obtain)
