The Role

• 
  Deliver primary, acute, and specialty care in home and community-based settings
• 
  Serve as a primary care provider where permitted
• 
  Manage ongoing treatment plans and patient panels
• 
  Order and interpret diagnostics to guide care decisions
• 
  Prescribe medications and therapeutic treatments
• 
  Educate patients and caregivers on prevention and chronic disease management
• 
  Collaborate with physicians and interdisciplinary care teams

Requirements

• 
  MSN or equivalent advanced practice degree
• 
  1+ year Nurse Practitioner experience
• 
  Active, unrestricted CA RN & NP license
• 
  Comfortable working in a mobile, field-based environment
• 
  TB clearance (or ability to obtain)
