The Role

• 
  Provide primary, acute, and specialty care in home and virtual environments
• 
  Evaluate, diagnose, and manage patient health needs
• 
  Order and interpret diagnostics to guide treatment decisions
• 
  Develop and implement individualized care plans
• 
  Prescribe medications and therapeutic treatments within scope
• 
  Educate patients and families on prevention and chronic disease management
• 
  Collaborate with physicians and interdisciplinary care teams

Requirements

• 
  MSN or equivalent advanced practice degree
• 
  1+ year Nurse Practitioner experience
• 
  Active, unrestricted RN & NP license
• 
  Comfortable in home-based or mobile care environments
• 
  TB clearance (or ability to obtain)
