The Role

• 
  Provide advanced nephrology care for patients with CKD and dialysis needs
• 
  Conduct comprehensive assessments, histories, and physical exams
• 
  Diagnose, treat, and manage complex renal conditions
• 
  Order and interpret diagnostics to guide clinical decisions
• 
  Develop and implement individualized treatment plans
• 
  Prescribe medications and therapeutic interventions within scope
• 
  Monitor treatment response and adjust care plans as needed
• 
  Collaborate with physicians on complex cases while maintaining clinical independence

Work Locations
Primary sites in Upland, West Covina, and Apple Valley
(Occasional travel to additional clinic locations as needed)

Requirements

• 
  MSN or equivalent advanced practice degree
• 
  2+ years Nurse Practitioner experience
• 
  Active NP board certification
• 
  Active, unrestricted CA RN & NP license
• 
  Nephrology certification or 1+ year nephrology experience
• 
  Adult/Geriatric or Internal Medicine experience preferred
• 
  Bilingual English/Spanish preferred (not required)
