Regional Home Care Nurse – Make a Meaningful Difference Every Day

In this full-time, direct-hire position, you will:
- Visit patients in their homes to provide skilled nursing care such as wound treatment, medication administration, and health assessments.
- Monitor and evaluate patient conditions, ensuring timely communication with physicians and care teams.
- Educate patients and their caregivers on managing health conditions safely and effectively at home.
- Document your visits and care plans accurately using an electronic health record (EHR) system.
- Collaborate closely with care coordinators, partner organizations, and interdisciplinary teams.
- Travel within your assigned region to visit patients.
Jamaica, Queens NY
What You Bring:
- Active RN license in New York State (unrestricted)
- Degree from an NLN-accredited nursing program (BSN preferred)
- At least 3 years of clinical nursing experience, including 1–2 years in home care or community health
- Strong critical thinking, time management, and communication skills
- Comfort with EHR systems and mobile documentation
- A reliable vehicle and valid NY State driver’s license preferred
- Willingness to provide weekend or evening coverage on occasion
- Ability to safely lift, move, and care for patients as needed
- Comfortable traveling between home visits and working in diverse home environments
- Competitive salary: $120,000 – $125,000 annually
