Description:

LPN - Full Time | Days & Evenings | Long-Term Care
Schedule & Shift Details
  • Must be able to start on a Tuesday or Thursday for full shift orientation
  • Full Time Days and Evenings (scheduled as needed)
  • 5x8 hours per week (40 hours)
  • Every other weekend (EOW) required
  • Holiday coverage required

Licensure & Certifications Required
  • Valid New York LPN Licensure
  • BLS certification — AHA or RQI only

Education
  • High School Diploma or GED required

Experience
  • Minimum of 1 year of experience as an LPN
  • Minimum of 1 year of experience in a Long-Term Care (LTC) or CNA capacity

Position Overview
  • Works under the direction of licensed nursing personnel
  • Performs direct and indirect designated resident care within the scope of practice as defined by certification
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit
  • Must demonstrate knowledge of the principles of growth and development of the life span
  • Must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs
  • Provides care for adult and geriatric patient groups

Job Functions Activities of Daily Living (ADLs)
  • Ensures resident ADL needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment
  • Responsibilities include: hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care
  • Documentation completed per protocol
Environment & Safety
  • Maintains resident environment and safety as outlined in the plan of care and in accordance with policies and procedures
  • Responsibilities include: answering call lights and alarms promptly, provision of safety mats and low beds, restraint use under the direction of Nursing, maintaining cleanliness of unit, behavior and fall prevention, and appropriate use of and monitoring for malfunctioning equipment
  • Documentation completed per protocol
Nutritional Needs
  • Ensures resident nutritional needs are met as outlined in the plan of care and in accordance with policies and procedures
  • Responsibilities include: meal delivery, tray preparation, adaptive equipment, feeding, and nourishments
  • Resident preferences and choices shall be honored
  • Documentation completed per protocol
Rehabilitative Needs
  • Ensures resident rehabilitative (restorative and maintenance) needs are met as outlined in the plan of care and in accordance with policies and procedures
  • Responsibilities include: range of motion, ambulation, transfers, proper body alignment, and use of assistive devices
  • Documentation completed per protocol
Resident Rights & Status Changes
  • Resident rights, refusals, and concerns are reported to the Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures
  • Changes in resident physical, emotional, and psychosocial status are identified and reported to the Charge Nurse promptly

Occupational Exposure
  • Occupational exposure to bloodborne pathogens is reasonably anticipated
  • Required tasks involve potential contact with blood, body fluids, or tissues
