Description:
LPN - Part Time Evenings | Long-Term Care Setting
Shift Details
- Part Time Evenings (2x8s or 3x8s)
- Every other weekend required
- Holidays as needed (3x8.0)
- All contracts must run through the holiday season
- HCP must be available for full shift orientation on a Tuesday or Thursday
Position Overview
- LPN - Part Time (24 hours per week)
- Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification
- Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit
- Must demonstrate knowledge of the principles of growth and development of the life span
- Must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs
- Provides care to adult and geriatric patient groups
Education Requirements
- Must have a High School Diploma or GED
Required Qualifications
- Active New York LPN Licensure
- BLS certification (AHA or RQI only)
- Minimum 1 year of experience as an LPN
- Holiday availability required
Job Functions Activities of Daily Living (ADLs)
- Resident ADL needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment
- Includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, and related tasks
- Documentation completed per protocol
- Resident environment and safety maintained as outlined in the plan of care and in accordance with policies and procedures
- Includes answering call lights and alarms promptly, provision of safety mats and low beds, restraint use under the direction of Nursing, maintaining cleanliness of unit, behavior and fall prevention, appropriate use of and monitoring for malfunctioning equipment
- Documentation completed per protocol
- Resident nutritional needs met as outlined in the plan of care and in accordance with policies and procedures
- Includes meal delivery, tray preparation, adaptive equipment, feeding, and nourishments
- Resident preferences and choices shall be honored
- Documentation completed per protocol
- Resident rehabilitative (restorative and maintenance) needs met as outlined in the plan of care and in accordance with policies and procedures
- Includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices
- Documentation completed per protocol
- Resident rights, refusals, and concerns reported to Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures
- Changes in resident physical, emotional, and psychosocial status are identified and reported to Charge Nurse promptly
Occupational Exposure
- Occupational exposure to bloodborne pathogens is reasonably anticipated
- Required tasks involve potential contact with blood, body fluids, or tissues
Required Skills
- Minimum one (1) year of experience in a Long-Term Care (LTC) or CNA capacity
