Description:
Licensed Practical Nurse - Full Time Day/Evening Rotation Position Overview
- Full-time LPN position with day/evening rotation schedule (5x8 shifts)
- Every other weekend and holiday coverage required
- Opportunity to provide direct and indirect skilled nursing care for assigned residents
- Work under supervision of Registered Nurse following defined policies and procedures
- Provide care appropriate to the age of patients served on assigned unit
- Demonstrate knowledge of growth and development principles across the life span
- Assess data reflective of patient's status and interpret appropriate information
- Identify each patient's requirements relative to age-specific needs
- Provide care needed by adult and geriatric patient groups
- Maintain collaborative relationships with nursing staff, physicians, and ancillary staff
- Maintain appropriate level of knowledge and clinical expertise through continuing education and in-service programs
- Collect data relevant to resident's physical and psychosocial condition on ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team
- Contribute to creation of developmentally age appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes involving the resident/family and healthcare team in this process while promoting a healthy, safe environment
- Recommend revisions to the plan of care, after evaluation of resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed
- Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice assuring customer satisfaction
- Maintain current knowledge and competency in Nursing Practice
- Participate in the mentoring/orientation of staff
- Consider factors related to safety, cost, and impact in planning in delivery of care
- Under the direction of the RN, provide leadership in the professional practice setting
- Demonstrate knowledge and competency
- Current NY LPN Licensure
- BLS certification through AHA or RQI specifically
- Minimum 1 year experience as LPN
- 1 year experience serving in an LPN capacity at LTC Facility
- Occupational exposure to blood borne pathogens is reasonably anticipated
- Required tasks involve potential contact with blood, body fluids, or tissues
