Description:
Licensed Practical Nurse - Full Time Day/Evening Rotation Position Overview
  • Full-time LPN position with day/evening rotation schedule (5x8 shifts)
  • Every other weekend and holiday coverage required
  • Opportunity to provide direct and indirect skilled nursing care for assigned residents
  • Work under supervision of Registered Nurse following defined policies and procedures
Essential Responsibilities
  • Provide care appropriate to the age of patients served on assigned unit
  • Demonstrate knowledge of growth and development principles across the life span
  • Assess data reflective of patient's status and interpret appropriate information
  • Identify each patient's requirements relative to age-specific needs
  • Provide care needed by adult and geriatric patient groups
  • Maintain collaborative relationships with nursing staff, physicians, and ancillary staff
  • Maintain appropriate level of knowledge and clinical expertise through continuing education and in-service programs
Key Job Duties
  • Collect data relevant to resident's physical and psychosocial condition on ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team
  • Contribute to creation of developmentally age appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes involving the resident/family and healthcare team in this process while promoting a healthy, safe environment
  • Recommend revisions to the plan of care, after evaluation of resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed
  • Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice assuring customer satisfaction
  • Maintain current knowledge and competency in Nursing Practice
  • Participate in the mentoring/orientation of staff
  • Consider factors related to safety, cost, and impact in planning in delivery of care
  • Under the direction of the RN, provide leadership in the professional practice setting
  • Demonstrate knowledge and competency
Required Qualifications
  • Current NY LPN Licensure
  • BLS certification through AHA or RQI specifically
  • Minimum 1 year experience as LPN
  • 1 year experience serving in an LPN capacity at LTC Facility
Important Information
  • Occupational exposure to blood borne pathogens is reasonably anticipated
  • Required tasks involve potential contact with blood, body fluids, or tissues
