Description:
LPN Part Time Position - Evening/Night Shift Rotations Schedule & Shift Information
- Part-time position with rotating schedule: 2x8 hour shifts one week, 3x8 hour shifts the following week
- Evening/night shift rotations
- Every other weekend (EOW) and holiday coverage required
- Current New York LPN Licensure
- Basic Life Support (BLS) certification - AHA/RQI only
- Minimum 1 year experience serving in an LPN capacity at Long Term Care (LTC) Facility
- Demonstrate knowledge and skills necessary to provide care appropriate to the age of patients served on assigned unit
- Demonstrate knowledge of principles of growth and development of the life span
- Possess ability to assess data reflective of patient's status and interpret appropriate information needed to identify each patient's requirements relative to age-specific needs
- Provide care needed by adult and geriatric patient groups
- Provide direct and indirect skilled nursing care for assigned residents
- Follow defined policies and procedures under supervision of a Registered Nurse
- Maintain collaborative relationship with other nursing staff, physicians, and ancillary staff
- Maintain appropriate level of knowledge and clinical expertise through continuing education and Inservice programs
- Ability to provide care to geriatric residents
- Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team
- Contribute to the creation of a developmentally age appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes involving the resident/family and healthcare team in this process while promoting a healthy, safe environment
- Recommend revisions to the plan of care, after evaluation of resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed
- Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice assuring customer satisfaction
- Maintain current knowledge and competency in Nursing Practice
- Participate in the mentoring/orientation of staff
- Consider factors related to safety, cost, and impact in planning in delivery of care
- Under the direction of the RN, provide leadership in the professional practice setting
- Demonstrate knowledge and competency
- Occupational exposure to blood borne pathogens is reasonably anticipated
- Required tasks involve potential contact with blood, body fluids, or tissues
