Now Hiring: Field Nurse Practitioner (Adult/Geriatric)
South Bay, CA | Mon–Fri |  No Nights, Weekends, or On-Call

$125,000–$175,000 + Bonus Potential

The Role


  • Provide in-home assessments and ongoing medical management

  • Manage acute, post-acute, and transitional care patients

  • Develop and execute personalized care plans

  • Order diagnostics, prescribe treatments, and educate patients/families

  • Collaborate closely with physicians and interdisciplinary teams

Requirements


  • MSN required

  • Adult or Geriatric NP certification

  • 1+ year NP experience (home-based or transitional care a plus)

  • Active CA RN & NP license

  • TB clearance (or ability to obtain)
