Your Role: As the Director of Social Work, you’ll be a key player in the planning and delivery of top-tier social services, ensuring that each resident receives individualized care throughout their rehabilitation journey. Your responsibilities will include:
  • Leading the Social Services Department, establishing goals, and setting policies that drive excellence.
  • Overseeing comprehensive assessments, care planning, and helping residents transition smoothly into their new environment.
  • Managing and mentoring a skilled team of staff, fostering professional growth and development.
  • Ensuring discharge planning begins at admission to promote long-term success and wellness for residents.
  • Engaging in occasional community outreach to strengthen our ties with local organizations and support networks.
What We’re Looking For:
  • Experience: At least 2 years of management experience in long-term care (LTC) settings.
  • Education: LSW or MSW is required.
  • Expertise: A strong understanding of State and Federal guidelines, with the ability to implement them effectively.
  • Leadership: Exceptional interpersonal and leadership skills, with a natural ability to mentor and support your team.
