- Schedule: Day 7:00am – 3:30pm; Evening 3:00pm – 11:30pm; Night 11:00pm – 7:30a

The CNA will follow assigned plans of care, assist residents with activities of daily living, and communicate important resident information to the licensed nursing team.

Responsibilities
- Follow the resident assignment sheet and individual plan of care
- Assist residents with activities of daily living, including meals, hygiene, and other personal care needs
- Listen carefully to shift report and prioritize resident care appropriately
- Communicate relevant resident care information to the licensed nurse throughout the shift
- Notify the licensed nurse when leaving the unit and at the end of the shift
- Communicate important information with coworkers to help maintain continuity and quality of resident care
- Complete all required ADL documentation by the end of the scheduled shift
- Follow facility policies, procedures, and resident care standards
- Minimum 1 year of CNA experience
- Valid New York State Nurse Aide certification
- Current physical examination and immunization records
- Ability to communicate effectively with residents, nurses, and other members of the healthcare team
- Reliable attendance and ability to follow assigned schedules
- Scrubs are required while working in the facility.
