Description:
CNA Per Diem Position Position Overview • Per Diem CNA position requiring 24 hours monthly including 2 weekend shifts • Must be able to complete full day orientation on a Tuesday or Thursday • Contract must run through holiday season • Minimum shift requirement: 24 hours (or 3-shifts) worked within a 4-week period, with at least 2 shifts being on weekends Required Qualifications • NYS Nursing Assistant Certification required • BLS certification (AHA or RQI) required • One (1) Year of previous LTC experience • Must be licensed for 1 year • One (1) Year experience in LTC or CNA capacity required Position Description • Under the direction of the licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on assigned unit • Must demonstrate knowledge of the principles of growth and development of the life span • Must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs • Provide care needed by adult and geriatric patient group Job Functions Activities of Daily Living • Resident's ADL's needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment • Including hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, etc. • Documentation completed per protocol Environment and Safety • Resident's environment and safety to be maintained as outlined in the plan of care and in accordance with policies and procedures • Including answering call lights and alarms promptly, provision of safety mats and low beds, restraint use under the direction of Nursing, maintaining cleanliness of unit, behavior and fall prevention, appropriate use of and monitoring for malfunctioning equipment, etc. • Documentation completed per protocol Nutritional Care • Resident's nutritional needs will be met as outlined in the plan of care and in accordance with policies and procedures • Including meal delivery, tray preparation, adaptive equipment, feeding, nourishments, etc. • Resident's preferences and choices shall be honored • Documentation completed per protocol Rehabilitative Care • Resident's rehabilitative (restorative and maintenance) needs are met as outlined in plan of care and in accordance with policies and procedures • Including range of motion, ambulation, transfers, proper body alignment and use of assistive devices • Documentation completed per protocol Resident Advocacy and Communication • Resident's rights, refusals, and concerns are reported to Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures • Changes in resident physical, emotional and psychosocial status are identified and reported to Charge Nurse promptly Important Notice • Occupational exposure to blood borne pathogens is reasonably anticipated • Required tasks involve potential contact with blood, body fluids, or tissues
