As a Social Worker, you'll be instrumental in helping residents manage and improve their physical, emotional, and psychosocial needs. Your expertise will guide residents through their care journey, ensuring they receive holistic support that improves their quality of life. Your responsibilities will include:
- Care Plan Development: Collaborating with residents and the care team to create individualized care plans that address their unique needs.
- Emotional & Social Support: Identifying and addressing the social and emotional needs of residents, helping them maintain their dignity and independence.
- Interdisciplinary Collaboration: Working alongside a skilled team of healthcare professionals to provide comprehensive, well-rounded care.
- Admissions & Transitions: Supporting residents through the admissions process, re-entry into the community, or transfers to other facilities.
- Education: Bachelor’s Degree in Human Services, Psychology, Social Work, or a related field.
- Experience: Prior experience in Long-Term Care or Subacute settings is required.
- Passion for Care: A genuine desire to improve the lives of others through compassionate care and support.
- Team Player: Strong communication skills and the ability to collaborate with interdisciplinary teams to ensure the best outcomes for residents.
