Position: Social Worker
Location: Queens, NY
Hours: Full-time, 9:00 AM – 5:00 PM, 35 hours/week

As a Social Worker, you will have the unique opportunity to advocate for and support elderly individuals facing complex health and social challenges. You will work alongside a passionate interdisciplinary team to help participants navigate both everyday life and the difficult moments that come with aging, illness, or disability.
Key Responsibilities include:
- Empowering Participants: Advocate and assist participants in overcoming personal and environmental challenges, helping them access community resources, and ensuring they get the support they need to live independently and safely.
- Comprehensive Support: Conduct psychosocial assessments, develop care plans, and provide emotional support for participants and their families, including crisis intervention and end-of-life care.
- Community Collaboration: Work closely with hospitals, skilled nursing facilities, and community agencies to ensure a seamless, coordinated care experience for participants.
- Education & Guidance: Educate participants and their families on navigating health services, government benefits, and community resources that enhance their quality of life.
- Continuous Improvement: Participate in ongoing education, peer reviews, and care plan meetings to continually improve your knowledge and impact.
- Education: M.S.W. from an accredited university.
- Experience: At least one year of experience in healthcare or working with elderly populations. Home care and/or fieldwork experience is a plus!
- License: LMSW or LCSW, with an active NY State Social Work license.
- Passion & Empathy: A genuine desire to make a difference in the lives of older adults and their families.
