About the Role: As an RN Care Delivery Field Supervisor, you’ll lead and oversee clinical services delivered directly to patients’ homes, ensuring the highest standards of care. You’ll work collaboratively as part of a dedicated management team, bringing guidance and support to field nurses and home health aides, and help deliver person-centered, high-quality care to participants.
What is Offered:
- Competitive Salary: Earn between $110,000-$120,000 annually, based on your experience and credentials.
- Full-Time, Flexible Schedule: Work Monday through Friday, 8:30 AM - 5:30 PM, offering a stable work-life balance.
- Prime Location: Conveniently based in Manhattan, NY.

- Supervise and support a team of Community Health Nurses (CHNs), Personal Care Aides, and Home Health Aides.
- Conduct joint visits and oversee care, ensuring compliance with individual care plans.
- Collaborate with the Interdisciplinary Team (IDT) to create and implement customized care plans for each participant.
- Provide hands-on education to participants and staff on best practices, safety, and healthcare topics.
- Take on a leadership role in new employee orientation and field visits, as needed.
- Education: RN and BSN required (or a minimum of 5 years in a healthcare setting); Baccalaureate degree preferred.
- Experience: At least 1 year in home care or community health settings and experience with frail or elderly populations preferred.
- Licensure: Active, unrestricted RN license in New York State.
- Technical Skills: Proficiency in computer systems and managed long-term care insurance experience is a plus.
- Strong organizational skills with a commitment to patient-centered, compassionate care.
- Ability to supervise and guide staff to meet the highest standards of patient care.
- Comfortable traveling between sites and working in various community environments.
