Schedule: Monday to Friday, 8:00 AM – 5:00 PM
Hourly Rate: $85.00

What You’ll Do:
- Deliver comprehensive medical care as a primary care provider to a diverse patient population, with a strong focus on geriatric and community-based care.
- Lead and support an interdisciplinary care team, ensuring coordinated and patient-centered treatment.
- Provide medical services in multiple settings including the treatment center and patients' homes.
- Participate in care planning, utilization review, and quality assurance initiatives to optimize patient outcomes.
- Serve as a resource and mentor to clinical staff including nurse practitioners, RNs, and trainees.
- Engage in patient assessments, case conferences, family meetings, and community health outreach.
- Support ongoing staff education and participate in shaping care delivery programs.
- Graduation from an accredited Physician Assistant program.
- Active and unrestricted NYS Physician Assistant license and current NCCPA certification.
- NY DEA registration and BLS certification.
- Board certification in Internal Medicine or Family Practice.
- At least one year of experience with frail or elderly populations (or a commitment to training if less than one year).
- Background in geriatrics or palliative care is highly preferred.
- Strong communication skills and a commitment to team-based care.
- Ability to meet standard physical demands of outpatient and community-based care.
- Medically cleared and fully immunized for direct patient care.
- Must hold active Medicare and Medicaid provider credentials.
