Location: Brooklyn, NY
Duration: 2-Month Contract (with potential for extension)
Schedule: 9:00 AM – 5:00 PM, 35 hours/week
Compensation: $55 to $70 per Hour (Depending on Experience) Plus Benefits

What You Will Do
As a Physical Therapist, you’ll be at the heart of patient care, providing therapy to help participants regain function, alleviate pain, and prevent future disabilities. You will play a key role in improving quality of life for individuals in a PACE program, including home visits and evaluations at various sites such as nursing homes and community settings.
Key Responsibilities:
- Conduct comprehensive evaluations and reassessments, developing and implementing personalized treatment plans.
- Deliver physical therapy treatments both in the clinic and in participants’ homes or other facilities.
- Collaborate with interdisciplinary teams to create and adjust care plans that meet the needs of each participant.
- Educate patients, families, and staff to promote functional independence and safety.
- Maintain up-to-date documentation of participant care, treatment plans, progress, and statistical data.
- Mentor and supervise Physical Therapy students, assistants, and volunteers.
- Ensure patient safety and follow strict health and safety protocols.
- Actively participate in care team meetings and training programs to enhance clinical and team effectiveness.
What We’re Looking For
To succeed in this role, you should have:
- Education: Successful completion of a Physical Therapy program from an APTA-accredited university or equivalent.
- Experience:
- At least 1 year of experience as a Physical Therapist.
- Experience working with elderly or frail populations (geriatric care experience preferred).
- License: Active New York State licensure as a Registered Physical Therapist.
- Additional Skills:
- Knowledge of PACE regulations is preferred.
- Proficiency in electronic medical records (EMRs) like Care Compass.
- Bilingual skills are a plus, depending on participant demographics.
- Strong communication and interpersonal skills to build rapport with patients and staff.
- Ability to travel to different patient sites, homes, and clinics within the Brooklyn area.
