Position Overview
As the Nurse Practitioner Supervisor, you’ll play a critical role in coordinating participant care alongside Physicians, community primary care physicians, and an Interdisciplinary Team (IDT). In this full-time role, you’ll be the liaison between participants, healthcare providers, and plan staff—ensuring holistic, person-centered care plans are implemented, while facilitating timely, preventative care services. This position requires local travel to client facilities in Ridgewood, Queens, NY
What You Will Do
- Provide direct supervision of Nurse Practitioners, manage clinical staff, and ensure the quality of care across nursing homes and PACE sites
- Perform annual wellness assessments for participants, adjusting care for those with higher risks, frailty, or complex needs
- Facilitate same-day urgent care, timely assessments, and transitions of care upon hospital discharge
- Collaborate closely with PCPs to co-manage high-risk participants, proactively identifying and addressing gaps in care
- Ensure accurate clinical documentation to reflect participants’ health status and outcomes
- Oversee and coordinate home care services, tailoring care plans to individual needs with follow-ups for comprehensive outcomes
- Minimum of 2 years in clinical nursing practice (home care or care management preferred)
- At least 1 year of experience with elderly or frail populations
- Valid NY State Nurse Practitioner license, DEA Certification, and AANP or ANCC Board Certification
