Job Title: Dietitian (Contract Assignment)
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Contract Length: 5-12 Weeks

Your Role and Responsibilities
As a Dietitian, you’ll provide individualized and group nutritional services that support participant health and independence. Your expertise will guide comprehensive care plans, ensuring they meet participants’ dietary and therapeutic needs.
Key Responsibilities Include:
- Conduct initial and follow-up assessments of participants' nutritional status through in-person and telehealth consultations.
- Create individualized nutrition plans in collaboration with the interdisciplinary team, ensuring all recommendations align with participants' needs and goals.
- Monitor participant progress and revise plans of care as needed, keeping the team informed of updates.
- Provide nutrition education and counseling to participants, caregivers, and staff to ensure understanding and adherence to dietary recommendations.
- Travel locally to participants' homes and facilities to deliver direct care and perform evaluations.
- Collaborate on group activities, meal planning, and educational initiatives with the Nutrition Services Team.
- Document all findings and care plans accurately within the organization’s systems.
- Support quality assurance and compliance efforts, ensuring adherence to professional and legal standards, including HIPAA.
What You’ll Need to Succeed
Education:
- Bachelor’s Degree from an accredited Dietetics program.
Experience:
- At least one (1) year of clinical nutrition experience.
- Experience working with a frail or elderly population preferred (appropriate training will be provided if necessary).
- Home care experience is a plus.
Licenses & Certifications:
- Registered Dietitian Nutritionist (RDN) certification through the Commission on Dietetic Registration.
- Certified Dietitian Nutritionist (CDN) licensure in New York State.
Skills & Attributes:
- Strong adaptability and problem-solving abilities.
- Exceptional communication and teaching skills to engage with participants, caregivers, and staff.
- Proficiency in using office software for documentation and reporting.
- Ability to interpret and apply professional and regulatory guidelines to daily work.
Additional Requirements:
- Must be medically cleared for communicable diseases and have up-to-date immunizations before direct participant contact.
- Flexibility to travel locally to participant homes and care facilities.
