Compensation: $50 - $60 Per Hour (Depending on Experience) Plus Benefits
Schedule: Monday to Friday, 8:30 AM - 5:00 PM (37.5 hours per week)

What You’ll Do
In this role, you’ll be an essential member of the interdisciplinary team, focusing on high-quality patient care in a collaborative and supportive setting. Here’s a snapshot of your responsibilities:
1. Collaborative Care and Communication
- Coordinate with the Site Medical Director and team on Infection Control and Surveillance activities.
- Educate participants on PACE services, respond to queries, and address pharmacy-related concerns.
- Deliver medications and treatments as prescribed, perform diagnostic tests, and handle procedures such as venipuncture, catheterization, and immunizations.
- Assess, report, and document patient conditions, medication reactions, and lab results to ensure effective care.
- Maintain a safe environment by adhering to National Patient Safety standards.
- Document all medications, manage inventory, and ensure supplies are readily available and compliant with safety standards.
- Ensure smooth operation of the DTC, from maintaining equipment to coordinating mail-order prescriptions and restocking inventory.
- Engage in participant-centered activities to foster a welcoming and supportive experience.
We’re looking for a compassionate and qualified RN who is self-directed and has a strong commitment to participant-focused care. Here are the qualifications we seek:
Education & Certification
- Graduate of an accredited Nursing Program (BSN preferred), with completed infection control coursework.
- Active NYS RN license and required certifications in Phlebotomy/Venipuncture, IV Therapy, EKG, and Basic Life Support (American Heart Association or American Red Cross certified).
- Minimum of 1 year clinical experience in home health, acute care, or a nursing home setting, with experience working with the elderly population.
- Strong interpersonal skills and a professional attitude.
- Ability to work independently, efficiently, and tactfully within a team.
- Comfortable with computer use, including electronic records and email.
