We are looking for an experienced Community Health Nurse to join the team. This temporary assignment that runs from June 30 to September 30, 2025, Monday-Friday, 8:30 AM - 5:00 PM.
What Is Offered:
- Competitive Salary: $67 per hour, based on your expertise, experience, and credentials.

In this role, you will make a direct impact on the lives of our residents by:
- Assessing and addressing patients' health needs holistically, from physical to emotional well-being.
- Collaborating with primary care providers and creating tailored care plans.
- Administering medications and treatments while independently performing skilled nursing techniques as needed.
- Teaching and supporting patients and their families for a healthier, safer lifestyle.
- Schedule is Tuesday through Saturday or Sunday through Thursday.
- Candidates will be making 4 to 5 in home visits per day.
- Education & Experience: 1+ years of clinical nursing in home health, acute care, medical-surgical, or critical care.
- Licensure: Current NY State RN license and a valid driver’s license.
- Homecare experience is required.
