We’re seeking a Clinical Assessment Manager to join a leading senior care provider and make a meaningful impact on the lives of seniors in Manhattan, NY. This full-time role, Monday-Friday 8:30 AM-5:30 PM, offers a unique opportunity to work with a dedicated healthcare team that champions resident well-being, dignity, and autonomy.

As a Clinical Assessment Manager, you’ll lead in-home and virtual assessments for participants, ensuring the quality and accuracy of care plans while upholding PACE program standards. You’ll partner with intake, care delivery, and quality teams, performing assessments, evaluating medical documentation, and providing detailed insights for high-quality participant care.
Key Responsibilities:
- Conduct in-home assessments and reassessments for participants using the UAS-NY system
- Work closely with intake and care delivery teams to ensure accurate initial and ongoing care planning
- Educate potential participants and referral sources on PACE program benefits and eligibility
- Promote quality improvements by managing documentation accuracy and collaborating with the quality review team
- Maintain and update participant health records in the EMR, ensuring compliance with CMS/PACE guidelines
- Minimum of 5 years in care management, utilization review, quality assurance, or discharge planning
- At least 2 years in managed care settings (MLTC, LTHHCP, MAP, or PACE preferred)
- 1 year of experience with elderly or frail populations
- Current, unrestricted NY State RN license
