Account Manager – Community Outreach & Engagement (Healthcare)

Full-Time | Monday–Friday, 8:30 AM–5:30 PM
Compensation: $75,000 – $90,000 annually
Location: Bronx (travel required)

Please Note: This position requires a valid driver's license and a reliable vehicle. Candidates must be comfortable working in the field approximately 80% of the time, with frequent travel across the NYC metropolitan area.

Our firm is proud to partner with a leading healthcare provider to recruit an Account Manager for a full-time, direct-hire opportunity. This organization delivers a comprehensive care model designed to support older adults living independently in their communities.

As part of their growth, they are seeking an experienced and community-driven Account Manager to help expand awareness, build referral partnerships, and support enrollment in a highly respected Program of All-Inclusive Care for the Elderly (PACE).

This role is ideal for someone with a healthcare business development or outreach background who is passionate about making a difference and thrives in a community-facing role.

Position Overview
In this impactful role, you will represent a well-established healthcare program throughout the NYC metro area—educating providers, caregivers, and community partners about its benefits and helping guide eligible participants through the referral and enrollment process.

You’ll work closely with teams across outreach, intake, and provider relations to ensure referrals are processed smoothly and in full compliance with regulatory standards. From leading presentations and coordinating events to building long-term relationships with community leaders, your work will be central to the organization’s mission.

Key Responsibilities

  • Cultivate relationships with referral partners, healthcare providers, and community stakeholders
  • Deliver clear, compelling education about the PACE program through presentations, events, and one-on-one meetings
  • Support community engagement initiatives and assist with event planning and logistics
  • Respond to inquiries and manage referral follow-up, supporting the intake team throughout the process
  • Track performance metrics and maintain detailed records in CRM and internal systems
  • Partner with the marketing team to advise on messaging and promotional opportunities
  • Maintain up-to-date knowledge of Medicare, Medicaid, and PACE-specific compliance guidelines

What We’re Looking For

  • Valid driver’s license and access to a reliable personal vehicle (required)
  • 2+ years of experience in healthcare outreach, business development, or provider engagement (PACE, MLTC, MAP, or homecare experience preferred)
  • Strong presentation and public speaking skills; comfortable engaging with diverse audiences
  • Ability to work independently, manage a dynamic schedule, and prioritize effectively
  • Familiarity with community-based long-term care services for dual-eligible and elderly populations
  • Proficiency with Microsoft Office; CRM experience preferred (Care Compass is a plus)
  • Must be willing to travel up to 80% of the time across NYC metro area

Working Environment & Physical Expectations
This is a field-based position that involves community travel, event attendance, and in-person engagements. Candidates should be comfortable with occasional evening or weekend hours and be physically able to carry outreach materials and set up event spaces.

Why Apply?
This is more than a typical outreach or sales role—it’s a chance to truly make a difference in the lives of older adults and their families. You’ll join a mission-focused team and contribute to a model of care that is changing how communities support aging with dignity.

If you’re looking for a role where your skills and passion for community engagement can directly impact lives—and where your contributions are truly valued—we encourage you to apply.
