About the Role: As an RN Care Delivery Field Supervisor, you’ll lead and oversee clinical services delivered directly to patients’ homes, ensuring the highest standards of care. You’ll work collaboratively as part of a dedicated management team, bringing guidance and support to field nurses and home health aides, and help deliver person-centered, high-quality care to participants.

What You'll Do:
  • Supervise and support a team of Community Health Nurses (CHNs), Personal Care Aides, and Home Health Aides.
  • Conduct joint visits and oversee care, ensuring compliance with individual care plans.
  • Collaborate with the Interdisciplinary Team (IDT) to create and implement customized care plans for each participant.
  • Provide hands-on education to participants and staff on best practices, safety, and healthcare topics.
  • Take on a leadership role in new employee orientation and field visits, as needed.
About You:
  • Education: RN and BSN required (or a minimum of 5 years in a healthcare setting); Baccalaureate degree preferred.
  • Experience: At least 1 year in home care or community health settings and experience with frail or elderly populations preferred.
  • Licensure: Active, unrestricted RN license in New York State.
  • Technical Skills: Proficiency in computer systems and managed long-term care insurance experience is a plus.

  • Strong organizational skills with a commitment to patient-centered, compassionate care.
  • Ability to supervise and guide staff to meet the highest standards of patient care.
  • Comfortable traveling between sites and working in various community environments.
